Those who will be automatically rejected are those with criminal convictions, no fly orders or those suspected of terrorism, assuming you don’t fall into any of these categories, you should be fine. The ESTA application will cost you $14 in local currency, and this is non-refundable, even if you get rejected.
Do I Need One Every Time That I Go?
Once you are granted your ESTA, it will remain valid for 2 years before you need to renew it. During this two years you are free to travel in and out of the country as you wish.

When Should I Apply?
You should apply for your ESTA as soon as you know that you are going to the USA. The reason for this is that should you get rejected, arranging an appointment at the embassy, getting there and fixing the situation could take quite some time, so its better to get everything in order first. You may lose some time on your ESTA if you apply early, but it will be better than being rejected and not having time to fix it.
